Behavioral health billing, done for you

For psychiatrists, psychologists and therapists. Your clinicians document the session. We code it (session minutes, add-ons, crisis and telehealth rules), check it and see it paid. You keep your EMR and see every claim live.

Live for behavioral health practices.

The rules we code and check before a behavioral health claim goes out

Session minutes pick the code, and each payer writes its own telehealth rules.

Table 1Behavioral health billing rules
RuleWhat it meansWhat we checkWho decides
Diagnostic evaluations90791 is the evaluation without medical services; 90792 adds medical services, such as prescribing. Billed at the start of care, and again only after a long break or a significant change. One Medicare contractor treats about 6 months as a long break.A repeat evaluation goes out only with the documented reasonMohenara codes from your clinician's note; your clinician documents the reason for a repeat
Psychotherapy by timeTime with the patient or family member picks the code: 16 to 37 minutes is 90832, 38 to 52 is 90834, 53 or more is 90837.The code matches the documented minutes; start and stop times or total time on the noteMohenara codes from the minutes in your clinician's note
Psychotherapy with a medical visitWhen a prescriber does both, the visit is an E/M code plus 90833, 90836 or 90838. Both must be significant and separately identifiable. The visit level is chosen by medical decision making, not time, and visit time does not count toward the therapy minutes.Add-on code only with an E/M code; therapy minutes counted apart from the visitMohenara codes from your prescriber's note
Interactive complexity90785 is an add-on for a session made harder to deliver, for example by a parent or guardian, an interpreter or play equipment. Never billed alone, never with crisis codes, and not with an E/M visit unless psychotherapy is billed too.90785 only with an allowed base code and the reason in the noteMohenara codes from your clinician's note
Crisis psychotherapy90839 covers the first 60 minutes and 90840 each further 30. They stand alone: not with 90791, 90792, 90832 to 90838 or 90785.No other psychiatric codes on a crisis visit; time documentedMohenara codes from your clinician's note
TelehealthMedicare pays behavioral health telehealth in the patient's home anywhere in the US, by video or audio only. Place of service 10 for the home, 02 elsewhere. From 2028, Medicare requires in-person visits, with some exceptions: within 6 months before a new telehealth patient's first visit, then at least every 12 months. Other payers set their own telehealth rules. Each payer's written telehealth rules: place of service, modifiers and audio-onlyMohenara codes from your clinician's note
Who can bill MedicarePsychiatrists, clinical psychologists, clinical social workers, nurse practitioners, physician assistants, and marriage and family therapists and mental health counselors, each within their state license. Each claim goes out under a clinician type Medicare pays for that serviceYou keep each clinician's enrollment current; Mohenara bills under it
Timely filingMedicare: one calendar year from the date of service. Other payers set their own. Each payer's deadline tracked; aging claims worked before itMohenara

Medicare contractors publish local rules for psychiatry and psychology services; we check the ones for your state.
